STEP 5- Robotic Prostatectomy: Bladder Neck Transection


By far, one of the most complex and difficult to master aspects of robotic radical prostatectomy is the division of the prostate from the bladder. This is so due to several reasons, which include the pitch of the camera with the tissue, the assymetry and contour of the prostate, the potential for a median lobe and the location of the ureteral orfices. Optimal dissection includes the creation of a small bladder neck opening with no injury into the prostate capsule to comprimise cancer control.Once complete, the posterior plane of the prostate is accessed. In our favored technique, the pre-dissected seminal vesicles from step 2 allows easy recognition and grasping of these structures to commence the nerve preservation.
